Forms in Revver gives you an easy way to create digital forms for employees, clients, or customers to fill out. Every submission is captured as structured data you can search and filter, and Revver automatically generates a unique PDF file for each response — fitting neatly into Revver's file-centric ecosystem.

Access & Feature Requirements
Forms is the new home for form creation and management in Revver, replacing the standalone Form Fill template workflow for new forms going forward.
My Forms - Shows the forms you have personally created.
All Account Forms - Shows every form created by every user on the account. This section requires an enabled feature on your user (currently tied to Template Management, with a dedicated Forms Management feature coming soon).
Your existing Form Fill templates are not affected and will continue to appear in their own table alongside the new Forms tables.
The Forms Management Page
The Forms management page uses Revver's updated table component, giving you filtering and sorting on every column.
Form Statuses
Each form has a status that reflects where it is in its lifecycle:
Draft - The form is still being built and has not been published.
Published - The form is live and able to accept responses.
Archived - The form is no longer active. Archived forms can be deleted if no longer needed.
Table Actions
Selecting the actions menu on a form gives you the following options:
Duplicate - Create a copy of an existing form.
Send - Distribute the form via link or direct email. See Forms - Distribution for details.
Edit - Open the form in the Builder.
Preview - View the form as a respondent would see it.
Archive - Move the form out of active use.
Delete - Permanently remove the form. Only available once a form has been archived.
Form Details Side Sheet
Selecting a form opens a details side sheet with the following information:
Link - The unique, shareable link for the form.
Destination - The location where response PDFs for this form are stored.
Description - An optional description you can add to the form.
Form ID - A unique identifier for the form. This ID is stamped on every response and output file, making it easy to trace a file or response back to the form it came from.
Date Created and Created By
Status
Responses - A count of total submissions received.
Fields - The number of fields on the form.
Version History - A record of changes made to the form over time.
Viewing Responses
To view submissions for a form, select Responses from the Forms management page. This opens the Responses table for that form.
Responses Table
The Responses table shows, for each submission:
The date and time the response was submitted.
A unique Response ID, used to help track that specific submission.
The value entered for each field on the form.
Like other tables in Forms, the Responses table supports searching, sorting, and filtering.
Response Side Sheet
Selecting an individual response opens a side sheet showing all fields and values for that submission. This view can be easier to scan than reading across a row in the table.
Viewing Response Files
Every submission generates a unique PDF file, stored at the destination configured for the form. To see every file generated by a form:
Open the Responses table for the form.
Select See All Files in the top right.
Because every response PDF is tagged with the form's Form ID, this shows every file generated from that form, regardless of where it was stored. You can also navigate directly to the destination folder to view the files there.
