Step-by-step guide to creating a new form using the Forms Builder in Revver

The Forms Builder is where you design a new form in Revver — from branding and layout down to the individual fields respondents will fill out. This article walks through the full building process.

Creating a New Form

From the Forms management page, select the + icon and choose New Form. Enter a name for your form to continue into the Builder.

Pages

A form can be broken into multiple pages.

  • Select the page icon to add a new page.

  • To delete a page, select it, then use the option in the builder side sheet.


Branding

By default, the Builder populates your form with your account's existing name and branding. This can be customized per form:

  • Image - Select the image to replace or remove it. An image is not required.

  • Account Name / Banner Title - This can be removed if you don't want a title banner on the form.

  • Color - Choose from a default set of colors, or use the custom color picker for an exact brand match.


Adding Form Elements

Forms are built by adding elements from the element panel. Available elements include:

  • Field - A metadata-driven form field (see below).

  • Header - A text header. Options include text size and alignment.

  • Paragraph - A block of descriptive text. Options include bold/italic styling, text size, and alignment.

  • Signature - Planned for a future release and may not be available at initial launch.

  • New Page - Adds another page to the form.

Fields & Metadata

Forms are tied to your account's metadata. When you select New Field, you are selecting from your existing metadata fields rather than creating a free-standing form field. This ensures that every value a respondent submits is captured both in the generated response PDF and as Revver metadata, so it can be used in search, workflows, and other automated processes.

Selecting New Field opens a modal listing your metadata fields, where you can:

  • Select which fields to add to the form.

  • Create a new metadata field on the fly, if you have the appropriate enabled features and permissions.

  • Reorder fields — the order set here is reflected on the form itself.

  • Toggle fields as required in bulk.

Metadata fields configured as dropdowns in Revver appear as dropdowns on the form. This gives respondents a preset list of values to choose from, rather than typing a value in freely — helping avoid the data integrity issues (misspellings, inconsistent entries, and so on) that can occur with free-text entry and cause problems later in metadata-based workflows, routing, or filing.

Field Settings

Selecting an individual field on the form lets you configure:

  • Label - The text shown on the form for this field. This can differ from the underlying metadata field's name without affecting which field it's mapped to.

  • Mapped Field - Which metadata field this element is linked to. This can be changed after the field has been added.

  • Description - Optional helper text shown below the field, useful for clarifying what value should be entered (for example, "This should match the legal name on file with your state").

  • Required - Whether the field must be filled out before the form can be submitted.


Layout Options

You can choose whether fields on the form are stacked or wrapped. This setting is best reviewed in the Preview page rather than the builder canvas.

Undo & Redo

The Builder supports undo and redo, so changes made while designing a form can be reversed or reapplied as needed.



Preview & Submission Settings

The Preview page shows how the form will look to respondents, with the option to toggle between a desktop and mobile view.

Submission Destination

Select the default location where response PDFs for this form should be stored. You can also configure a naming convention for the generated files; by default, Revver appends the date and time to each file name to help keep it unique.

Thank-You Message

Optionally, add a custom message to display after a respondent submits the form — for example, thanking them for their submission or providing next steps or a link for more information.



Saving Your Form

From the Builder, you can:

  • Save as Draft - Save your progress without making the form live.

  • Save and Publish - Make the form live and available to accept responses.
